Starting March 3,2023 Derby City DPC now offers DOT Physical exams at our clinic for members and non members from Michael Lovelace, MD. Membership is not required to get a DOT medical exam. We can also bill your company directly. We accept credit card, debit card, and cash.
The federal Department of Transportation requires medical certifications for all commercial drivers participating in interstate commerce. If you drive a commercial vehicle regionally or nationally, you must regularly pass a physical medical exam that is submitted. Check with your employers or the Department of Transportation if your driving does not include crossing state lines.
DOT Medical Exam Specifics
Each exam consists of two sections:
- Dr. Michael Lovelace will work with you to fill out a medical report. This includes information about your medical history, including surgeries, medications, and medical conditions. The process may also include your doctor asking questions to fill in any blanks in your medical history.
- The truck driver’s physical exam will include a blood pressure check, your weight, height, vision and hearing test (whisper), along with a urinalysis.
Important Information for your DOT Medical Exam
- Please bring your medical records. If you have MyChart or access to a patient portal that will provide the information we need. If you need help with a medical release form please call us at 502-324-5416.
- Do not stop or skip your medications. Your blood pressure will be checked so avoid smoking or caffeine before your appointment.
- You will need to provide a urine sample to check for glucose, protein, and specific gravity.
- Bring your valid driver’s license – either commercial or individual.
- If you usually wear contacts, bring along your prescription glasses to the exam.
- If you are hard of hearing and using hearing aids, don’t leave them at home. You might even bring extra batteries.
- Bring a complete list of medications from your pharmacy.
